My worst fear came true. My application package has been returned. I just picked it up a few minutes ago. The reason was, we forgot to write "PRESENT" in two places on my wife's forms (current address and personal history). I could not believe that I overlooked those when I had written 'Present' on my forms. I wasted 2 weeks of time and $17.50 for postage.
Anyway, let's make this thread to inform other people for the reasons the applications can be returned, so others can learn from our mistakes.

Please always use this checklist before sending:
1. Application fees Draft/cheque has NOT been expired or close to expire 2. Signature in all forms are current with current date 3. I797 and I94 forms are in packet if H1B is not stamped if stamped then copy of H1B stamping 4. No column should be left even if not applicabe write --NA-- even in Government, Miliatry service questions. 5. PCC's are not expired and original 6. US $ 465.00 and Canadian Dollar 550.00 for applications fees 7. Employment History, Address Histroy, dates are properly filled and till current/present 8. Immigration Photos are different than Passport photos especially after Sep-11 attacks so make sure photos are 35MM * 45MM sized with white background.

Thanks guys. I am sending my application again tomorrow. There is a problem though. My home country PCC expired yesterday and not sure if I need to request a new one and wait for it OR am I going to be fine with the old one as they have already seen it? Thanks.

I would send it as it is. Just make sure you include the original letter from CIC requesting correction. That is how they'll know they has your application b4 now.

Hi Nix, On the bright side, they did not ask you about the photos and the envelope:-) Re. the PCC, I would send what you currently have, esp. since it they know it is a resubmission. Good luck! Hi Nix,
My application was received on the same day and the MO has not been cashed. I moved to a new apartment yesterday with a forwarding address. I just want to know if the returned application was by USPS regular mail? Did they put the mail in the mailbox or delivered to your house/apartment as it would not fit the mailbox?

Thanks again guys. Mythdot, the mail was in my mailbox. My mail box is pretty big. It was NOT delivered to my home. It is a big yellowish/brownish envelope (9 inch x 13 inch-I measured it for you) and I think it is USPS mail as there is no way to tell other than it says "United State Postage" and they have paid $2.87 to send and it is postmarked Apr 17th.
