CHC REQUEST LETTER COPY

Hi

I have one quesion.

While sending the medical reports to London , do we have to forward CHC request letter copy along with medical reports. The reason is I forgot to hand over the copy of CHC request letter to medical center.

( in reqest letter , they nomally mention what they required, in my case RPRF draft and Medical request)

Hi,

In my case the medical reports has been forwarded from clinic side but forgot to attach the request letter copy, if this become a problem , this is my concern.
