Just want to make sure about supplementary information forms...
1. Residency Questionnaire ( 1 Page)
2. Supplementary Information Form ( 4 Pages)
3.Education Background/Work Experience Questionnaire (N/A) As i dont have dependents over 22 years of age.
Am I right?
Are u filing forms on ur own or CHC has asked you to fill these out? Did u get a letter asking specifically for these forms to be filled out? If they did then stick with that. The reason I am asking is becoz my residency questionare was 4 pages and there were 3 separate supplementary forms . Hope it helps. Good luck |
