Does anybody know i can go about and changing my daughters last name to my husbands now???? She is only three.When she was born her biological father never wanted to give her his last name so when filling out the forms i gave her mine. Recently i got married to my wonderful husband and he wants to give her his last name which i have no problem with ...but now her biological father wants to give her his since he recently found out i got married...never before he ever mention giving her his last name until now....but the reality is i don't want her to have her fathers last name ...i want to provide her with my husband since he has done more for her than her own father.....so yeah...does anyone have any insists to this???????

If the child HAS BEEN BORN IN ONTARIO, you will be required to contact the Office of the Registrar General for an alternative process:

Toll-free: 1-800-461-2156 or
Toronto: 416-325-8305

If the child HASN'T BEEN BORN IN ONTARIO, you will need to fill out this form:

thank you for the information.  I will give them a call to verify what other information i need. She was born in Canada.
