Change in family compostion before SendingPassport (Please guide me)

Please guide me

Actually today i got a letter from canadian embassay for passport request for stamping of visa for canadian immigration but from our last corrospondence with the embsaay there is addiotion of a baby in my family composition on 30 May 2009.In the letter it's mentioned i donot need to send my passport if there is any change in family composition.
If i will add my baby right now what will be procedure and how soon it will be completed and also please tell me what form i need to fill from cic website for adding my daughter.

1.  You have to submit an amended IMM 008 to show the child, you and your spouse amended IMM 5406s
copy of the birth certificate, copy of the passport bio data pages, $150.00 and then wait for medical instructions.

Do i need to send Schedule 1: Background/Declaration [IMM 0008 SCHEDULE 1] and Additional Family Information [IMM 5406] with all the mentioned documennts in the reply or i need to send all the forms.
