I am currently working in Canada and have gathered close to 24 months of experience. I do not understand question 19 in the application. It says to fill in my identity card number. Will I fill up my SIN number or drivers license number given by Ontario goverment?

What should I write in it , Social Insurance number or print the number instead?

Also do I have to notarize all the photocopies?

If your country doesn't issue identity card, put N/A.  Read the instructions, I doubt think any docs require notarizing.
