Hi, thank you for taking the time to read this.

I have just turned 28 in November,

I was about to go to the Canadian embassy in Tel Aviv to renew my passport which expired a month ago.

But i realized that i misplaced my Citizenship card!

I cannot find it!!!

I currently have:
-Quebec Driver license
-Commemoration certificate of Canadian citizenship (issued 1995)

I do not have Medicare, due to living outside of Canada for the last 8 years.

What exactly can i do to receive my passport ASAP?

Please help!

Would really appreciate a step by step guide!

Do you have the old passport?  I do not know if what you have is enough to get your passport asap.  I am sure you have some other ID to show that you are really you with the same name and birthdate as the holder of the passport.

As for replacing your citizenship card, you could apply for that through the embassy.  Here is how to apply: http://www.cic.gc.ca/english/citizenship/proof-how.asp
