CAIPS/FOSS request

I have searched for CAIPS/FOSS request form and found it;

but I am not sure which office to send it through my spouse. They have provided  addresses but I am confused about which one should we send the request.

Plus, why do I need to send a consent when my wife who is my sponsor is going to request for CAIPS along with FOSS? I guess I have to send the consent letter anyways, so, a letter on a plain paper with my signature authorizing my wife to request and receive CAIPS is enough or is there any form for giving consent? I have came across one more form on CIC website which is a consent form to release personal information to the authorized individual.

I wonder if I have to use this form in order to give consent and authorizing my wife to request and receive CAIPS on my behalf. Can we both request for my CAIPS and her FOSS through same IMM5563B form? or do we need to send separate forms with seperate CAD $ 5.00 fee for each?

FOSS is for inland
CAIPS is for outland

secondly the consent form is consenting for your wife to request the information.

weird but thats the way it works.

Caips must be requested ffrom the canadian citizen or PR, and will be sent to a  canadian address.

Thanks for your response but I guess no body here has ever actually requested for CAIPS or FOSS here to tell me exactly what to do...i know CAIPS are for outland and FOSS for inland and it has to be requested by a canadian citizen however my questions still remain; Which office should I send the request to? The form i have paste a link of is the correct form for giving consent? Can a person request for CAIPS and FOSS through same form?
