what boxes should be ticked (Checked) in the IMM5563B, Access to Information and Personal  Information Request Form. In the first BLOCK of the application form UNDER Access to Information Act ($5.Fee)  and Privacy Act (No Fee).

I am confused about "Received Copies of the record" and "Examine copies of records in a government office".
which box shall i check in both sides. shall i check both boxes or only ONE "Received Copies of the record" ..........please help me
