Can someone share the procedure to get the CAIPS notes.

Someone in Canada who is a citizen or permanent resident there, can request for CAIPS for you. Go on the following link to download the form. Send a consent letter signed by you to authorize the person in Canada to request for your CAIPS. Hope this helps.
