when I received my meds it was only one form per family member then the DMP added 2 more 1 for the lab test and one for the Xray maybe thats what you mean |

As @ Glowingheart said, DMP will have these forms and add it to your Section A and send everything to Ottawa. Just call up a DMP and go for medical!! Good Luck! HI Guys

Thanks for your kind information.I took an appointnment witih DMP.They are asking me of if i record of immuziation and vaccine record from previous.I said them i don't,and they told me i need to have that in the appointnment day.Did anybody took immuziation and vaccine in medical check up time.

*Unless original documents are specifically requested,please submit certified true copies of supporting document .All police certificates must be original.
this means i just need to submit medical right?Do they might call for interview?They didn't mentioned anything for interview?do you have any idea>

They did not ask anything else with Medicals. (btw they had ask for University Transcripts in sealed envelop in Dec 2011 before medicals, which I sent in Jan 2012, it was a pain to acquire them from two different Universities  ). I did not receive PCC request, because I had submitted PCC from 3 countries where I lived, along with my application to buffalo. |
