Birth certificate is a must. Fix all the discrepancies legally (through court / consult a lawyer / re-apply for Birth Cert mentioning the Date of Birth Mistake etc) and then apply.

Original birth certificate of both principal applicant and spouse is required. All dates should be matched with birth certificate.
