BC PNP: Information Release Form

Hello Guys,

I need your help with the BC PNP ''Information Release Form''.
In the signature section they have asked for 'Signature of Witness'. who's signature would that be?(employer/friend/family member) or do we have to leave it blank?
I will be applying from outside of Canada.
