IMM 0008:

please explain for me the last sentence is that mean this barcode page must be placed on the top of each application like travel information , additional family information, economic class 3 , and so on or not

You require just one. Put it on top of your whole application package.

so I have to put page 5 of 5 on the top of IMM 0008 or on the top of checklist owr what

The bar code page will be 1st page on your complete application package....so that when CIO opens the package s/he can see the bar code FIRST.

Don't worry too much....you will not be declined because of the order of the pages.....but try to be as systematic as possible with your complete package.....you don't want to irk your Visa officer and be in his bad books.
