you need nothing to explain just attach the refusal letter which bears your ref no. and if they want to verify from us embassy they will do it on their behalf. You just explain that you have over looked the question by human possible error and thats it.

Okay that I will explain but do they need a proof of the explanation, and reference number/case number of the application etc.

in my case H1B Company, H1B Petition Number, Refusal Letter etc?
