Access to Information requests should be directed to: Access to Information and Privacy Division Citizenship and Immigration Canada Narono Building 360 Laurier Avenue West, 10th Floor Ottawa, Ontario K1A 1L1 Telephone: 613-946-0953 Facsimile: 613-957-6517 How to order your immigration file notes There are two ways to obtain FOSS or CAIPS notes. If you are physically in Canada, you can obtain them for free under the Privacy Act. If you are overseas, someone else (a Citizen, Permanent Resident, or someone physically present in Canada) will have to obtain them for you under the Access to Information Act. This can be a relative, friend or sponsor, or a commercial service. For this there is a $5 fee (more if you use a commercial service), and you will have to provide consent. Forms for this are available on the Citizenship and Immigration Canada website. Payment can be made by credit card. (2011) Completing the Access to Information and Personal Information Request Form Choose "Receive copies of the records" For applications being processed overseas, choose "CAIPS (information abroad)". For applications being processed in Canada, choose "FOSS (information in Canada)". (Normally, it is not recommended to order "IN CANADA FILE (FOSS included)" since this requires that the paper file is taken out of storage and may affect processing times. The FOSS notes alone are usually sufficient.) The "Requester's reference no." can be left blank - that is used by representatives who are ordering FOSS notes for clients, who may have their own reference number for the file in question. Where to send the request For the address or fax number to send your request to, see Addresses for Personal Information Requests on the CIC website (mailing addresses only) or alternatively InfoSource(mailing addresses and fax numbers). FORMS: 1. Request Form http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf2. Consent Form http://www.cic.gc.ca/english/information/applications/release-info.asphttp://www.cic.gc.ca/english/pdf/kits/forms/IMM5475E.pdf3. retest din ako sa urine dati, had to fly back sa doctor just for the urine test alone. Inom kalang super daming tubig para ma ok yan. Dont think so medical is any hindrance. I've read it somewhere unless extreme siguro.

just thought anyone would be interested...
Info on caips notes part 1 GENERAL INFORMATION ON CAIPS NOTES
What is CAIPS? CAIPS stands for Computer Assisted Immigration System. It is part of the Electronic Data Processing (EDP) system used by the Canadian Immigration offices to process visa applications outside Canada.
When to get your CAIPS file? If you have been invited for an interview, get your CAIPS file to better prepare for the interview. If you feel that your application is taking much longer than usual, or you have not heard anything from the visa office for a long time, then you should consider getting your CAIPS file to check your case status. If you have any question regarding the application or you are just curious about your application
What kind of information is available in CAIPS file? CAIPS contains your all immigration information. 1) The status of your case and required documents (received or not, valid or invalid, etc) 2) Visa officer's comments on your case 3) Quesitons that you will be asked in the interview (if an interview is required)
How long does it to get my CAIPS file? From three weeks (at least) to 36 business days (at most).
What is the difference between CAIPS and CIC's e-cas ? CAIPS contains much more information than the information e-cas does. In most cases, CIC's e-cas only contains no more than three lines of information while a typical CAIPS file has 7 pages.
Will the request of my CAIPS file delay my Canadian Immigration application anyway? Absolutely not. To request your CAIPS file does not affect the processing of your Canadian Immigration application because your CAIPS file is requested & sent directly from CIC headquater in Ottawa while your application is processed in Port au Prince. Immigration officers in the Consulate or Embassy who review your application do not know of your request.
deciphering CAIPS notes One thing you should note is that dates are always provided in the format DD-MM-YYYY. So 10-03-2005 is March 10, not October 3 as it would be in the US date system.

of what I know as long as it is not a contagious disease hindi naman siguro. Ako I will be sending my app to my hubby today. Parang i felt too attached sa application hehehe at ang end result ang gulo-gulo ng room ko puro papers everywhere. Ngayon I need something to do to keep my mind off the app. hehehe
same here, the very first day na send app namin, thats all i thought of whole day sa work..
