Applying MY wife during my Case process.

Dear Experts,
I am want to apply my wife with me, any My file is still in process in CHC London. As per thier 2nd AOR they asked if there is any change so please let us know.
So My Question is what are the steps how can I send my wife's documents to them Because I am cofused on following points:
1-Do I Need to send my wife's Documents CHC london or CIO Office ?
2- where I will pay My wife's fee.
3- and What are the forms do i need to fill.
Please let me know the proper steps and I need expert's Guide .

you Mean again all my documents including my emloyer , bank etc etc.
Or just Forms ?
