Applying for Work Permit CONCURRENT to Permanent Residence

We are about to submit a Spousal PR Application - from within Canada - and have read that you can submit application for a Work Permit concurrent with PR Application.  Which Work Permit Application/Form do you use?  I thought I found it (the only one I could that referred to sending it in form 'within Canada' but ... it was for caregivers.  I'm looking for a regular (non caregiver) work permit application that one submits with PR Application.  Thanks in advance -
