APPLYING FOR PR (LIVE IN CAREGIVER)

Hi,

I am doing the paperworks for my PR application. Can anyone assist me with the following questions:

a) where can i get the statement of earnings showing hours worked and deductions made by employer (i do have 1 previous employer )
b) what is this record of wages and deductions?
c) does the option c i requested from CRA covers my question a and b?
d) do i have to send the T1?
e) do i have to ask for the T4 summary from my previous and current employer?
f) do i need to submit IMM 0008DEP and IMM5476 even if its not applicable for me? or i have to send them a blank copy?
f) i spend 1week vacation in the US and 1 week in the Philippines..they were both paid (i am allowed 2weeks vacation per year)..will they still be counted on my 24months employment requirement? how will cic knows thaat i covered the 24months employment requirement? do i have to submit a time sheet?
