(1) Initially the whole process (though what exactly IS happening -- Nobody knows) was predicted / assumed by Indira ... that is I think in the first 10 pages of this thread... will try to get that.....

Yup Anil, i am the same person. I got the package back as i recalled the package literally from the door step of CIO. Its with me and i see some other issues such as the one i have inquired above.

Please let me know if any of you guys have any answer to the question.

Guys, one last minute question before i send out my application today. My spouse name is "Tahmina Akter Ratna". Now, we need to provide only Last name and Given Name in the application. I was thinking to put Last Name = Ratna and First name = Tahmina. But one of my friend who has similar situation says that, Given name should be  = Tahmina Akter. I was thinking Akter as middle name. But there is now way to put middle name in the application.

My friend is telling me that its cruical that i put both Given and middle name at Given name as there is no middle name. I have checked my previous US visit visa in which i see they do exactly follow the method as my friend suggested.

Seniors, please help me to finalize my application. I am confused!!

Her passport doesnt segregate names. Its written as full name. No specific first or last name. That is the dillema!

The simplest (& I think correct too!!) answer is to fill the form as per the exact details in the passport. The passport is the key for filling up the forms....... and if there is any change / difference in the name when we compare PP with other certificates like birth, marriage, degrees etc. then it's better you give a reasonable / genuine explanation for this difference. There will be change of name after marriage too!!!
If there is only one name in the PP...put the same in Given name (and not in the family name...or middle / last name)...

Moreover, you can put different names in a particular box (made specifically for this)..... . It is under Schedule 1,
Sl # 3  - You can include all her names...which are there in her certificates...and which you feel may create some problem / confusion in future!!! .................Hope this helped

I recalled my package only few hours before it was supposed to be delivered on 28th morning. It was sent back to me on the same date and then i got it back on 2nd october. Its not unlikely that we et package from USA and canada in 3-4 business days. Its actually quite usual. And Sunday is a working day in my country.

However, i did pay around $200 for this recall procedure.
