Schedule 1 Background Declaration, and the Additional Information (the one where you have to list your family members) - as I recall it's only those 2 forms which require signatures, although its advised to also sign and date any explanatory letters you include.

5620, Fee payment form, I thought that the entire row is reserved for "official use only"...sad

I just reviewed my documentation, and I found out that I have not (correction, signed it, but not dated) it either Sad - this is ridiculously misleading.

I think with this I can forget any hopes of 1122 this year

You do not need to sign the fee payment form. Only the background declaration and additional information forms.

My payment form was credit card...

Thanks Anil, I confused it means the applications will be processed within 12 months( processing time one year for FSW) as I did spot somewhere post june 26 applicants will be processed within one year but now I am unable to find out where I did it. Anyway good luck for all.Perhaps time flies so fast!

Ah ok sorry my apologies, I got this wrong, I have just re-checked the fee payment form. If you pay by credit card you do need to sign the form (but not so if you pay by banker's draft). We paid by banker's draft so just completed Section A of the form. I knew we hadn't needed to sign the form and hadn't remembered the extra section for credit card payers.

Looking through Section B of the form, it is indeed very misleading. Clearly the credit card section needs to be signed, but the date is on the same line as the "For official use only" box. BUT the official use only box is greyed out.

So my feeling is they want you to date the Section B on the form if you are paying by credit card. However, I think anyone whose credit card has been successfully charged need not worry about this now, since it has worked. Failing to date the signature may or may not be a problem I think as long as the transaction goes through electronically OK they will probably not be that bothered about it.

Anil did your credit card charge ok? If so then I think you are going to be ok. It would certainly help if they made the form clearer.

For 'CC' payment, you need to sign/date it - apparently user 'ska' app has been rejected because of lack of date. I have also signed it but not dated it Sad

I just see that I had included a copy of my cc, and signed/dated it too. If only I did not read that as the 'for official use', i would have dated it too.

With 1122 I don't even have an opportunity to resend it! This sucks big time

Like I said, if they successfully took your credit card payment I think you will be fine. But any future applicants may wish to double check that they signed and dated Section B.

Very sorry to hear this. I have heard of people being re-placed in the queue after having payment problems, so if your application does come back send it back with a fresh payment form and a letter explaining why the confusion occurred, and request that they consider your application from its original received date. Will be worth a try!

Hehe yes I think I half-replied to 2 different posts!

Anyway, for the IMM forms:-
- Schedule 1: Background/Declaration [IMM 0008 SCHEDULE 1]
- Additional Family Information [IMM 5406]

These are the only two main forms which have to be signed by all FSW1 applicants.

Additionally, Section B of the Fee Payment Form has to be signed and (we think) dated if you are paying by credit card (for payments using money order or banker's draft, Section B does not need to be completed at all and the form does not have to be signed or dated).

Are you referring to the bit about "If you do not sign your application form" etc?

I had a similar fear but I asked a former visa officer on the British Expats forum who confirmed it is only the forms which have a box to be signed.
