double-check with your DMP before going in-person ! Some DMP's have the forms and some don't!

Right you are....I wonder why they make it so difficult for the applicants and why can they not streamline the procedure  Huh Huh Huh....this is like hitting your head against a brick...oh no no no....I should say "Iron wall"

Anyways....one can download the forms from the Internet.

Kummy29 & Mitali, I do not know about the case with other countries, but with NDVO... I am certainly sure what Harjot says is correct... They have stopped sending the forms now and the DMP provides them when we go there...

So, Dr. Bhatia, nothing to worry... Just go with the Cover Letter and the Medical Instructions and rest will be taken care of by the DMP... Best luck!!  Smiley

Thats GOOD to know that NDVO is gradually reducing paper work. Grin Hope they reduce the processing times further, esp the Return of PPs, and respond to the queries in a systematic manner.

I too did not receive the forms. I broke the head and heads of mitali, bhakti , nokiadesire and they were so supportive.
The bottom line is that ndvo is not mailing the forms to us anymore. We have to mention this to the DMP and they have the forms and will provide you at the time of examination. Nothing to worry! All the very best for your tests and hope you have a speedy processing thereafter!! Smiley Smiley

I wish I could wake up your VO and demand your Visas..... Smiley...but its not in our hands. Wait a while longer till the water reaches your nose and do the last effort .....ask him to send your PPs back as you have no other option left....hope that will stir some common sense.
