In question #1, under "other name(s) used", do i need to put my nickname and middle name here?

How about in Question Letter "C" (Coming into Canada), Am I going to put Vancouver,BC (port of entry) as my original entry to Canada or Edmonton,AB  because that is where i landed...  Thanks again! :-)

my husband put Vancouver as port of entry because that is where he entered Canada
