I am a bit confused ... My name written on medical letter is abdullah nasir warraich... Whereas my name is abdullah saif warraich...nd there is no list attached of doctors ... They have form on which a number is written for medical appointments 051 111 466 472

If "nasir" is not your maiden name or other name then you should fax CHC Islamabad. Quote: " Please, check the accuracy of the personal information. If there are any significant errors, please advise us by fax or letter indicating your file number and the errors. We will send you a corrected form and you should then destroy the incorrect Section A. If we have included your maiden name or other name(s) in Section A, you may still proceed with your medical examination." |

Bro my middle name is saif .. where as nasir is mentioned here... Also documents of medical are for immigration visa..no list of doctors attached...first line is in order to determine whether you meet the medical admissibilty criteria in canada's immigration and refugee protection act you and your eligible family members must undergo medical examinations

IRPA is a set of rules for everyone entering canada, it's the correct documents. Your only problem is your name. Fax them your file number and the error and don't proceed for medical until you receive a corrected Medical form. Also mention about the list of DMPs missing. |

Dont painc, there is a procedure to fix this up! Just relax and follow the rules they have described. Inam has given you an excellent peace of advice. Follow it and things would be fine. Get the fax and email ready to be sent to them. I have heard that if you keep the subject of the email "URGENT", then it is given priority. Any how calm and down and calculate your next steps! You can proceed to medical .. its already written in the document which is provided by you .. that one can proceed to medical if there is change of name... Can u tell me their fax number ...

I also got the same package bro,to book appointment just call on the number given. and yar mera name bhi unho nay medical letter par ' SHEHRYAR ZAHID CHAUDHRY" likha hay par shehryar kay spellings galat hay,sahi spellinh hay sheryar,,should i be worried? |

but according to inam's qoutes he says any significant change but its just spelling mistake in SHERYAR but my ZAHID CHAUDHRY is written correctly,so I think its fine,what u say inam? and i also did not got any list,it just written that call on the number an take appointment ,which i did and got appointment. |

ok,I think i should wait for others to comment on it, and then deicide,but i think there is no need of it as my FILE NUMBER,DOB and other spellings are fine,,,,and yaar ab wo doctors ki list nahi dety,they just give one number and we have to take appointment.

I think you should fax CHC, what if they repeat the same mistake in stamping your visa? so better fax them. |
