Application returned, asking for this document

I applied to the Buffalo office of the Canadian Consulate. My application was returned asking for this particular document (Appendix C):

"current or expired status document indicating the date you were admitted and the validity of the documents issued on that date"

I'm on H1B status in the US. Does anyone know which document I need to provide here? It maybe I94, but that's a separate option on the form indicating what they need from me.

 I think you should send  a copy of the I-94 and maybe a copy of your visa
