trade unions; and
professional organizations.

Do I understand it correctly that your Physician's Syndicate is actually a group of doctors who share a practice/office space/duty calls?  If this is the case I do not think it falls under the umbrella of a Professional Organization.  A professional organization would be something akin to the Canadian Medical Association which WOULD be included in this section of your form.  I hope this helps.

Professional organization or bodies are those who set the ethics, standards, and issue licence or authorization to practice under a specific profession (authorized and protected by government legislations).
Like The College of Physicians and Surgeons of Ontario is the regulatory body for Physicians and Surgeons in Ontario which operate under a Federal Authority. it sets standard and ethics to practice for  physicians, it hear the misconduct cases and can bar guilty physician from practice in Canada. Normally, every country has similar bodies for medical and other professions. So, if you are a physician from a country, you are suppose to be a member of professional organization to do public practice and you should mention it they way Dr. .Hasib demonstrated.
