Appendix D and the IMM107EFC

Hello,

I have all the forms ready to send to my common-law sponsor but i can't find the Appendix D and the  IMM107EFC form on the internet.

Is it the doctor that gives me the form?
Or is it the immigration office that sends it to me after?
Is there anywhere where I will be able to find this form?

I went to the doctor and he said to me that he doesn't have de IMM107EFC form.

But the problem is, how i ask for it, and where????
