Hey Everyone...

My wife just had her Medical done today in Mississauga, Ontario.  Are they supposed to do anything with the Appendix C form?  They actually gave it back to us and said that we have to include it with the IMM1017 Medical Report (copy 2). 

Just wondering if anyone can confirm this.

yes thats right , u need include that in ur file and send all to the processing center its nothing wrong in that thats what we all did too
