Greetings to you all!

I hope this is the right place to ask this question. This is concerning Appendix C and medical exam for my spouse (USA permanent resident) whom I'm sponsoring.

1. Does anyone know how much does the medical exam cost (she has no healthcare/insurance coverage)?
2. Is Appendix C the only document she needs to bring to the Designated Medical Practitioner?

Many thanks,

Joseph & Magnolia

1. It varies from DMP to DMP. It can be as low as around $175. I've also seen as high as $550. It's typicallly cheaper in Canada (especially in the large cities).

2. You also need to bring photographs of the applicant.

My husband did his medical in Miami and it cost $380 - even though they told me it would be $410 over the phone - obviously the cheaper the better Smiley

The receptionist told him to bring the Appendix C and 4 passport photos, when he got there the doc said he needed 5...UGH...anyway the doc signed the paper and just told him to add the photo after.

Id bring more pictures than they ask for just in case.

Also - no insurance will cover this exam, so just call around, but in the end if you have to drive 5 hours to get a cheaper rate it might be worth to go closer to home, gas is too expensive.

Good Luck. My husband said it was an easy exam and not as invasive as he really thought it would be.

1. Ours costed about $200, it depends where you do it.
2. We had to bring along 2 passport sized photos, the applicant's passport and the immigration file number.

Our DMP also asked for vaccination records in addition to photos and the appendix, so I would call the DMP you are using a just ask.

Ours cost 250 EUR in Stuttgart.
