In the instructions for IMM 1294 form, it is written to "Place the barcode page (page 5 of 5) on the top of each individual package".
What does individual package mean?

That barcode is specific for one particular form filled. If more than one forms (in case of more than one applicant) are being filled, the bar code has to be attached on top of each of the forms.

Since only you are applying for a Study Permit, you just need to place it on top of IMM1294. I suggest you keep the form filling guideline provided on the website with you. The link is given below:

I am not sure if the following form is required.

Secondly in the IMM 5645E Family Information form it is mentioned to write 'all names in English and your native language'. So, should I write the names in Urdu (native language) as well?

Download the "Application Guideline Form - IMM5269E" and refer to it when filling in the information of different forms.

IMM5269E - Page 12:

IMM5257 - Application for a Temporary Resident Visa
IMM 5645 - Family Information Form

Both of these are a must until and unless you are from a country, whose citizens are Exempted and do not require a TRV for entrance into Canada and Pakistan is not one of them.

I think IMM5257 is required only if you have selected 'yes' in questions 3 to 7 of IMM1294E. I am saying this on the basis of page 21-22 of the guide.

I agree. I cross checked the whole guideline and the documents as well as the CIC website. IMM 1249 Covers all questions of IMM5257. So you if 3 - 7 dont apply to you, you dont need to submit IMM5257.

However, "Supplementary Information Form" is a must requirement. That covers your family composition.

2. Supplementary Information Form is not needed. Instead, fill out and submit IMM5645E (Family Information Form) with your application.

  .. I had supplementary with me but the lady at the submission counter said "Ye aap rakh lein, is ki zarurat nahi" ..
