anybody knows how to accomplish affidavit of denial @ Philippine Consulate?

hi everyone..
I am currently working on having my police clearance done, however
my name was a hit, and worst of all I have the same name with someone who has a pending case in
our local municipal trial court. so NBI manila sent me information to get a few details regarding the case and to
accomplish an affidavit of denial here in our consulate.
anybody who has experienced this and can share any idea rgarding how i should go about it ?
I have been calling the Philippine Consulate here in Vancouver the whole morning but I cant get through.
