Hello, what advice would you give to a returning Canadian naturalised citizen who has been out of the country for over a year and is pregnant? Would she be required to buy private insurance during the 3 month wait period?

She's not required to, but I would recommend it.

Not required but it's a question if she can even get insurance to cover pregnancy related expenses.  She can get some kind of emergency insurance for sure.
