You look for a designated doctor in your region (see link below) and make appointment. For the appointment you'll need 2 photos for each person (passport photos should be ok). photo-ids (passport or driver license), the forms that CIC sent to you and most important cash or check... For my medicals the doctor did not accept credit card...
In MA I paid for the medicals us$350 plus 25 dollars for express service, which includes tracking number and usps express mail...
The exams took around 1 hour and they did blood pressure, chest x-ray, urine and blood test..

Hi Nix,

Not to worry at all!! I had the same concern with photo background and also did the same as you for the check and photos (envelope).

I got my application returned from CIC with a note to update dates and sign, but there was absolutely no mention of anything regarding the photos or anything else. They are probably a bit flexible about such things:-) Anyways, I resent my application and just found out from my bank that the check was cashed. So now, I just have to wait for AOR (fingers crossed).

If I may, I would like to ask you a question to get some ideas on timeline. After how many days of receiving the application did they cash the check?

I don't think anybody has reached till PR under H1B SRS program. Certain filipinos has reached this level but under employer driven stream and stamping from Manila.

It was the old process for H1B no longer available, they Change the rules I believe on Feb 19 2009

The lady at my bank (Wachovia) confirmed that the check was encashed. Then she asked me, would you like a print. On the back of the check is my File # Bxxxxxxxxx.

Even though I knew it was too early, I tried it out as 'Immigration File Number' on the 'Client Application Status' page, and the result was, as expected, not found.

Congratulations! At least now you can sleep well knowing that your application package will not be returned. They cashed the check in about 8 days from the date the received it. I will have to call my bank in about a week, may be next Thursday or Friday.
