An affidavit is a document on which the translator has sworn, in the presence of a commissioner authorized to administer oaths in the country in which the translator is living, that the contents of their translation are a true translation and representation of the contents of the original document.

Can this statement be a part of my translated peace of document, printed somewhere at the end of translation, or it has to be independent peace of paper?

  Afidavit is written on Government paper( stamp paper). No need to write those sentences. write like this,  I firmly affirm that.......

So you are saying that I do not need to send this extra government paper with my translated documents, and that it is enough to write this statement at the end of translated document?
