Hello:)

I'm being sponsored as a common-law partner, by my Canadian girlfriend. I have to fill out this form, right? And I have no wife or ex wife or children or...  just parents and siblings, whom I include?

What I really want to know is, in Section A it asks for my own details, my parents, and my spouse or common-law... do I put my Canadian girlfriend (sponsor) here?

Yes, you still need to complete it, this is where you list your relatives such as parents and siblings.  Yes, you list your common-law partner where it asks for details of your spouse/common-law partner.
