Additional Family Information (need help regarding filling it up)

I would like to know when spouse is going to fill in additional family information form, will she write down her name in Applicant section & main applicant's name in Spouse section?

Yes, when you fill in your Additional Family Information form, you write your name as the applicant and your spouse as spouse. When she fills in the form, she writes her name as the applicant and your name as the spouse.
