additional family information form

hey guys.
i am a bit confused,any help will be appreciated.
being the main applicant how to fill out my spouse additional family information form,i mean how to fill section A.
would it be my name in applicant line or my spouse name.
actually there are two seperate forms one to be filled by me n other one by my spouse.....so in this topic i am talking about my spouse one.

If you are the Principal applicant your name in the  Section A against  Applicant

The name of your spouse  against  Spouse  

If you have children  show in Section B - same in both forms

Your brothers/sisters only in Section C

In the other form for spouse also same but

in section C only the brothers/sisters of  spouse.

On your spouse's form, put her brothers and sisters.
