This form you need only if you have included so many dependants in the one form that you have no space actually. You must declare as dependant only whoever is coming with you to Canada. And you will have to pay the fees for all of them

Dependents:

Spouse/Common-law partner,
Child
Adopted child
Grand child
Step child

If you have already listed 5 dependents,  then go for additional dependents form IMM0008DEP for each dependant.
