My dear. Forms to be submitted is only ADDITIONAL FAMILY INFORMATION. This is to provide CHC with updated data on the family and nothing else.

No offence dear but do u think that simple payment of fee/birth certificate/ picture submission by applicant and CHC will themselves fill out the form for applicant This may be the one of case but not a bench mark / procedure. I have shared the standard procedure and suggest to follow.
