Dear Raniasaleim..

Below is the detail of documents or actions that you need to take in order to comply fully with CHC requirements for new born;

1. NEW BORN Birth certificates (hospital and ministry of health). No need of notarization. Only copies can serve the purpose.
2. Passport of new born (and visa page if residing outside homeland)
3. Five photographs as per specification,
4. Additional family information forms (update) and signed in original
5. Demand draft of CAD 150/- (in the name of RECEIVER GENERAL FOR CANADA)
6. Information on height of new born at the time of sending forms.

Process immediately after the birth of child and inform CHC on earliest basis so they do not process your case with out this information otherwise it may take longer than normal due to delayed info

Send simple letter to VO immediately to update. Later, on top priority and at earliest, send rest of the details. Type of details and documents i have already mentioned in this thread

I had a baby last august and i added my daughter to the file, by submitting a cover letter ,her birth certificate, 4 passport pictures, copy of passport, as well as $150CDN draft for processing fee. Its a simple process.

Dear

please carefully note that you must  complete and submit those forms . without these forms with sumission, you are sure to be delayed  and even the case can be an unmanageable one . In my case, though I was delayed a lot for the bitrht of the new born , I was asked to submit those forms. so be advised that you complete , sign, and send those forms .
wishes for you.
