Actually stuck on "Name and address". Help

Background Info:
I'm Canadian sponsoring my American husband.
He lived in Pennsylvania until July 2010 when he found a job on the US/Canadian border.
Since July 2010 he has been living here in Canada and crosses the border daily to go to work.
He has a "Visitor Record" so that he can cross the border without issues until he get PR.

I'm trying to complete my application and I'm stuck on a couple of "simple" things... I'm sure that no one else gets stuck on Name and Address (except me) lol.

1) Family name - on "Application to Sponsor" under Sponsor Personal Details, it asks for "Full Name: exactly as shown on your passport or travel document".  Ok, here's the question, my passport still shows my maiden name but I have everything else in my married name.  Does this mean I'm to use my maiden name on the full application just because I didn't change my name on my passport yet?  Also, I could use my birth certificate but also shows my maiden name.  Does this mean that if I was to include my birth certificate (which of course wouldn't have my married name) I'd have to use my maiden name on everything on the application?  HELP. I'm so confused.

2) Residential address - When my husband (who was my fiance at the time) got a job on the US/Canadian border July 2010 a family friend let him move his belongings into a bedroom on the US side of the border.  However, he has technically never lived there... but for purposes of drivers license, work etc, this was his "residential address".  Since getting married and getting a Visitor Record he technically lives here in Canada and his belongings are no longer at that previous address in the US.  My question is this, on the application, does he need a residential address in the US or do we use the Canadian one? Does it matter?

Use what it says on the passport.

It depends, are you applying inland or outland? If outland, residential address is the US address, mailing is the CDN address. Inland it's all CDN.

We are applying "outland"... but again, he doesn't technically have a US address anymore.  *sigh*  I guess we will have to ask our family friend if we can use her address again.

My husband is the same. He stayed with his friend's parents. But from what I understand, nothing goes to that address. All mailings go to the CDN one.

Lots of people on this forum have used the actual Canadian address they were living at as their 'residential' address, even though they were applying outland and were in Canada as a visitor. Nobody has reported any problems doing this.
You can always attach a letter explaining the circumstances to that form.
