A third request for background and declaration form

The first document I sent had a couple of errors(I ticked some sections wrongly)

During all my submissions I put an additional page because my personal information expecially job experience is lengthy and can't be contained in the required section

The next time a request was made asking for same,I forgot to sign and I immediately sent a correct copy within 24hours(can't remember if I put the additional page,so so confused)

Another request has just been made

They laid empasis on "no gaps in personal history" and addresses"

I have steadfastly been on the process for 14months

As long as you corrected all errors in the final submission, and submit everything before deadlines, then you should be fine.
