1. Does a background check expire like the doctors exam??? If so when?

2. For the application it says to include photo's, but no where in the application does it say a number it only says a set. Does anyone know how many I'm suppose to send???

3. In my package recieved from the FBI, it included blank finger print card and a few papers. One paper is titled Authorization of Release Form. I have no idea what I am suppose 2 do with this paper or if i even need it. It says something about a 3rd party and notarization.

Yes, it does. From the CIC guide: "The certificate must have been issued no more than three months prior to submitting your application. If the original certificate is not in English or French, then you will need to submit both the certificate and the original copy of the translation prepared by an accredited translator."

I would assume 6, but I don't know for sure.

I don't know in this specific case, but usually an authorization of release is needed if you want them to send the documents to another organization, for example, if you want FBI to send the certificate directly to CIC, bypassing you, then you need to sign the authorization. But I'm not sure.
