1249 as Open Work Pemit?

Hi,

I'm trying to prepare an application for an Open Work Permit to send in at the same time as
my Inland Permanent Residency application in the Common-Law class.
I'm slightly confused as to how to fill in the
'Application to Change Conditions, Extend My Stay or Remain in Canada' (IMM 1249)
as an application for an open work permit.

I think box 'C' should be ticked at the head of the form and the personal information is straightforward enough but how would I go about filling in questions 10 & 11?
They don't seem relevant to my application other than stating the reason for the application
is as an open work permit accompanying my PR application.

Has anyone gone through this process? Could I ask how you filled out this form?
