Hi all,
They require the "Additional family information" form completed by
-principal application
-spouse

Does this mean I have to fill out two form, one for me and one for my spouse?
If so, in the form for my spouse, in section A, I will need to write my spouse's mother and father name, right?

yes, on yours you are the principal and on your wifes she is the principal.

Sorry I don't get it completely.
As I understand, it would be like this:

(I attach here the form so you might take a look to remind you how it looks like

In section A, they ask the names of the followings:
-Applicant
-Spouse
-Mother and father

If I (principal applicant) complete this form, it is easy.
But if my spouse completes this form, should I say like this?
- Applicant: her name
- Spouse: my name
- Mother and father: her parents' name.

yes that's right spring2009,
your spouse should write her name as the principal applicant in her forms.

Actually on te form the person filling is called only "applicant" NOT "principal applicant". Remember that you are both applicants but you are the principal one.
